- Exhibits a noticeable decline in hygiene or cognition
- Makes frequent and inappropriate phone calls to your office
- Seems confused about issues discussed in the past or has repetitive conversations
- Exhibits difficulty handling incoming mail or managing money
- Needs to maximize access to funding sources such as C.O.P. funding, Family Care, or LTC Insurance Policies.
- Client becomes a caregiver to their spouse or child
- Family is at a distance, in stress or in disagreement
- Client is unaware of community agencies or housing options
- Client needs to relocate
