Making this decision, and looking for a new setting for your loved one, is often confusing and overwhelming, and individuals and families can benefit from objective, professional guidance in selecting and arranging for residential care.
The care managers at Geriatric Support/Pathway Care, can help you to understand the wide range of options that are available, and identify those living settings that are consistent with needs, preferences, and finances. We will assist with facility tours, applications and related forms, as well help you with relocation logistics.
This may include: collaboration with senior move managers, moving companies, real estate agents, and other service providers as needed. We will also continue to work with you and your loved one, to ensure that the transition to their new home is a smooth one.
Adapting to a changed environment requires the establishment of friendships, routines, and time to become familiar with the physical environment. Our staff will assist you and your loved one to adjust to a new setting, providing ongoing support and monitoring as needed.
