PRINCIPALS
Miriam Oliensis-Torres, MSW, LCSW, C-ASWCM, Founder and President of Geriatric Support/Pathway Care, has over thirty years of experience serving older adults, adults with long-term care needs, and their families.
Prior to founding Geriatric Support/Pathway Care, as Geriatric Support Associates in 1988, Miriam was involved in a variety of innovative programs serving older adults, adults with chronic neurologic conditions, mental illness, and those with Alzheimer's Disease and other dementias. She is an experienced psychotherapist and psychiatric social worker, trained in marriage and family counseling, and specializes in working with complex family situations.
Miriam is a Fellow of the National Association of Professional Geriatric Care Managers, and a Certified Care Manager. She currently serves on the Board of Directors of the Milwaukee Aging Consortium, and a variety of national committees. She served on the National Board of NAPGCM for five years, and is a past member of the Board of the Midwest Chapter of NAPGCM and the Alzheimer's Association of Southeastern Wisconsin. She has served on numerous local, regional and national committees, and is a frequent presenter at community events and professional conferences.
Miriam received her BS in Rehabilitation Education from the University of Wisconsin-Madison, her MSW from the University of Wisconsin-Milwaukee, and holds an advanced case management certification through the National Association of Social Workers.

Angie E. Van Scyoc, BBA, CEO and co-owner of Geriatric Support/Pathway Care, is a seasoned professional experienced in assisting families address difficulties as a skilled and objective sounding board.
Angie specializes in working through complex issues related to legal, financial and real estate matters, often acting a liaison with service professionals in related fields of expertise.

CARE MANAGERS
Bonnie Nestor, MSW, ACSW, CISW
Bonnie received a BA in social work from Indiana State University in 1972, and her MSW from the University of Wisconsin-Milwaukee in 1978. Bonnie has over 30 years experience working in the medical social work field, primarily with older adults and adults with disabilities. She has worked in a variety of settings, including nursing homes, hospitals, outpatient rehabilitation and home health care.
Bonnie is a member of the National Association of Professional Geriatric Care Managers and the National Association of Social Workers, and has been a Care Manager at Geriatric Support/Pathway Care since 1993. Bonnie's extensive experience and counseling expertise give her the skills to help older adults, adults with disabilities, and their family members, find solutions to the many issues they face.
Patricia Treffert, MSN, RN
Pat has a long history in elder care, and a great passion for working with seniors. She holds a BSN and Master's Degree in Nursing Administration from Marquette University. Pat's experience includes nursing home administration, assisted living operations, director of home care and hospice, and managing an independent senior housing setting.
Pat is member of the National Association of Professional Geriatric Care Managers, an active community volunteer, currently serving as Chairperson of the Aging and Disability Resource Center of Ozaukee County, Board President of Ozaukee Interfaith, and services on the Nursing Advisory Board of Cardinal Stritch University. Pat joined Geriatric Support/Pathway Care in July of 2008.
Debbie Wakefield, MSW, LCSW
Debbie received her BSW and MSW from the University of Wisconsin-Milwaukee in 1978 and 1980, respectively. She has worked in a variety of settings including in-patient hospitals, employee assistance, out-patient clinics, and private practice. She was recently employed from 2003-2009 as a case manager with older adults and adults with disabilities.
Debbie has also been an active support group leader with the Alzheimer's Association for more than five years. She is a member of the National Association of Professional Geriatric Care Managers, and has been a member of the National Association of Social Workers since 1986. Debbie joined Geriatric Support/Pathway Care in February 2010.
Phyllis Blackburn, BA
Phyllis has a BA in Psychology from the University of Wisconsin-Madison, and began working in the field of aging in 1971. She was instrumental in developing many community-based services in Madison, and was a member of the initial staff of Independent Living, Inc. She was the first Coalition Coordinator from 1974-1981. Phyllis developed and directed the Milwaukee Community Service Bank at St. Ann Center for Intergenerational Care from 1998 - 2003, and has been with Geriatric Support/Pathway Care since 2004. Phyllis' interest in older adults began in college, and has been an integral of her personal and professional life over the years.
Jackie Ceille, MSW, CISW
Jackie received her MSW from the University of Wisconsin-Milwaukee, taking every opportunity available to focus on gerontology. She has over 25 years in the field as a social worker, coordinating programs and providing services to individuals across the lifespan. Jackie was first employed at Geriatric Support/Pathway Care beginning in 1995 for three years, and rejoined our staff in 2009. Jackie has a special interest in helping individuals with intellectual disabilities to continue to experience a sense of belonging and purpose as they age.
