How Does it Work

The personalized service you will receive from Geriatric Support/Pathway Care starts with your first phone call to us. We will take the time to learn about your situation, and discuss the ways in which we can be helpful to you. You will learn about our services and fees, and have the opportunity to ask any questions that you may have.

Once you have retained Geriatric Support/Pathway Care, you have access to the full range of care management services offered by our team:

• Consultation:

We will meet with you at your home or in our office, to discuss your situation and determine how we can work together, to meet the needs of your loved one.

• Assessment:

We will meet your loved one, in their home or current place of residence, to conduct an assessment of their situation including strengths and limitations and wishes and preferences. We will include an assessment of physical health including: current and past health issues, medical care, mobility, medication review and management and:

  • Functional abilities and limitations: ability to care for self, manage in their home, pay bills, drive
  • Cognitive status: memory, reasoning and judgment, communication.
  • Social interactions: family and support network
  • Living situation: home safety assessment
  • Legal & financial issues: health insurance, prescription drug coverage, eligibility for benefits, paying for long-term care, planning for the future/ powers-of-attorney

• Recommendations/Development & Implementation of Care Plan:


Based on the assessment, Geriatric Support/Pathway Care will work with you to develop a personalized plan of care to fit your family’s situation, taking into account strengths and limitations, wishes and preferences, and financial circumstances. If needed, we can help you obtain appropriate services including:

  • In-home and community-based care such as in-home caregivers, delivered meals, chore services, transportation, and adult day programs.
  • Alternative living arrangements, including retirement communities, assisted living settings, group homes, and nursing homes.
  • Assistance with obtaining appropriate legal/financial services, including: benefit eligibility, financial management and planning, and legal matters (power of attorney, guardianship).
  • Referral to specialized medical and behavioral health assessment and treatment resources.

• Follow-Up


Your individualized plan of care will be developed to meet your family’s needs today and adapted as circumstances change. Geriatric Support/Pathway Care will be available to provide ongoing follow-up, monitoring, consultation, support and advocacy to ensure that your loved one continues to receive high-quality, appropriate care, adjusting the care plan as the needs change.

Geriatric Support/Pathway Care Care Managers provide the following:

  • Ongoing monitoring, support, and advocacy
  • Coordination of medical and support services
  • Family support
  • Crisis intervention
  • Periodic care plan review and revision
