Concerned about a loved one?  Confused about what to do?

People are living longer, frequently with significant health challenges or care needs.  Families are no less devoted than they ever were, but in today's complex world, sometimes find it difficult to know how to best meet the needs of their loved ones.

The geriatric care managers at Geriatric Support/ Pathway Care are available to provide the guidance and support in situations such as those in which:

  • Families become aware of changes in their loved ones -- perhaps their memory isn't what it used to be, or there are questions about how they are managing their medications and meals at home -- but they're uncertain as to whether they should intervene or not.
  • There are significant concerns about the health and well-being of a relative, but are families are uncertain or confused about how to approach the situation.
  • There is an immediate need for care or assistance related to an illness, injury, or hospitalization, and families don't know where to turn.
  • Family members are actively involved, but finding that they are having difficulty balancing the assistance they want to provide to their loved ones with their other responsibilities.
  • Older adults are doing well, but family members who live elsewhere are concerned about "what if" -- what happens if my parent becomes ill, and needs someone I can trust to watch over them and ensure that they receive the attention they need?

 

In all of these situations -- and many others, the care managers at Geriatric Support/Pathway Care are available to provide support and guidance for those who are faced with immediate needs, as well as those who want to anticipate and plan for the future.
