Geriatric Support/Pathway Care was founded in 1988 as Geriatric Support Associates, one of the first geriatric care management agencies in the State of Wisconsin.
Geriatric Support/Pathway Care provides personalized consultation, assessment and care management services for older adults, adults with disabilities or chronic care needs, and their families.
Families come to us for many reasons. Some live at a distance and cannot be as involved with their loved ones as they would like. Others find that, as members of the “sandwich generation”, they are being pulled in multiple directions.
There are those situations in which family members want the best, but are not in agreement about how to handle changes as they occur, or how to plan for the uncertain future. At times, family members find that they agree that “something needs to change”, but don’t know what changes to make, or which resources will best meet their loved ones’ needs.

A Care Manager is an advanced degreed professional who specializes in working with older adults, adults with disabilities and chronic health conditions and their families. Care Managers help to identify and resolve the myriad short and long term challenges faced by individuals, families, and those who are assisting them.
Care Managers have extensive knowledge about costs, quality and availability of services in their community and can connect you with the services that will be right for you or a loved one.
